Homebase Medical is getting back to basics. Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes. We make house calls - enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.

We offer an array of medical and chronic care services, and we're backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.

Responsibilities include:



  • Configure and maintain Salesforce Health Cloud to drive business value.
  • Bring design thinking to create a Health Cloud vision that meets and exceeds operational expectations.
  • Ensure data accuracy and system integrity across integrations with EHRs, scheduling systems, and mobile field tools.
  • Design and maintain real-time dashboards and reports for operational users and leaders.
  • Manage user provisioning and ensure HIPAA-compliant access controls for clinical and operational users.
  • Partner with clinical informatics and analytics teams to support data-driven decision-making.

Qualifications and experience:



  • Bachelor's Degree in a health-related field preferred
  • A comparable combination of education/experience and/or training will be considered equivalent to the education listed above.
  • 3+ years of Salesforce Administration experience, including at least 1 year with Salesforce Health Cloud in a healthcare environment.
  • D365 expertise
  • Proven experience as a Product Owner or Business Analyst in an agile or scrum-based delivery model.
  • Salesforce Administrator Certification (ADM 201) required.
  • Solid understanding of value-based care models and the unique needs of home-based or mobile medical practices.
  • Strong grasp of HIPAA, PHI compliance, and security best practices.
  • Excellent communication skills with the ability to translate user needs into technical solutions.
