Nexus Health Multi Specialty, affiliated with McKesson Specialty Health and US Oncology, a leader in cancer care, is seeking an experienced Full-Time Pharmacist to work in our brand new 3 level facility located in Santa Fe, NM. SCOPE: Under general supervision, compounds and dispenses medications, drugs, and other medications according to professional standards and practices. Prepares and maintains all necessary records on prescriptions, patient charges and inventory. Consults with nursing and medical staff regarding prescriptions, orders, drug and pharmaceutical details, patient reactions, errors and complaints. Initiates and follows quality assurance and safety policies and procedures. Monitors compliance with all legal regulations and accreditation organizations. Supports and adheres to the US Oncology Compliance Program, including the Code of Ethics and Business Standards.


Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provides medication, education, and counseling to patients and their families
  • Assesses, plans, implements, and evaluates care for patients
  • Initiates and documents appropriate interventions; evaluates and records patient/family response and recommends therapies as indicated
  • Assess patient/family needs and provides education
  • Promotes wellness and recommends community resources to meet patient and family needs
  • Consistently incorporates evidence-based research into clinical practice
  • In-depth knowledge of reimbursement procedures and documentation requirements
  • As a subject matter expert, promotes/provides staff education
  • Check all orders for completeness of information.
  • Verify pathway status and document status in the EMR. If the regimen is off pathway, ensure a pathway exception has been approved by the regional medical director prior to treatment.
  • Verify compendia compliance and requests off compendia forms as applicable.
  • Notify business office of regimens that are a reimbursement risk.
  • Verify appropriate dosing and schedule of all medications.
  • Round dosing per Nexus Health policy.
  • Verify appropriate premedications are ordered. Add additional premedication's as directed by Nexus Health policy.
  • Adjust antiemetics per the antiemetic protocol and notify physician of any changes warranted.
  • Review most current labs and notify appropriate site personnel if needed labs are missing or dose adjustments are needed.
  • Verify a treatment plan is documented in the EMR and notify the ordering physician if the treatment plan is missing.
  • Notify business office personnel that the medication or regimen has been reviewed by pharmacy.
  • Review regimens for drug interactions with current list of medications-if found alert prescriber/pharmacist/or APP.
  • Make recommendations for alterative medication as needed or dosing modifications.
  • Check that an oral chemotherapy prescription has been sent to pharmacy with appropriate instructions.
  • Verify medications adhere to the therapeutic interchange policy. Contact the physician for needed interchanges. If the interchange is denied, the reasoning is to be documented in the pharmacy note.
  • Follow Nexus Health No Mix policy and other guidelines as directed by pharmacy administration.
  • Verify the appropriate primary diagnosis code is used for regimen entry. Also verify secondary and tertiary codes when appropriate.
  • Implement quality initiatives are directed by pharmacy administration.
  • Research drug information or other clinical needs as requested by physicians.
  • Prepare reports and monitoring activities as directed.
  • Document all activities and pending issues in the pharmacy note in the EMR.
  • Other duties as assigned

Qualifications

The ideal candidate for this position will have the following background and experience:

  • Bachelor's degree from an accredited school of pharmacy and state licensure.
  • Minimum three (3) years experience as a registered pharmacist.
  • Oncology background and/or board certification in oncology (BCOP) strongly preferred.
  • Current license as Pharmacist in the state of Texas
