RN-Transfer Referral Center Part-Time

Responsible for performing aspects of the Patient Flow Management process ensuring that transfers, patient placement and related activities occur in a timely, safe and accurate fashion. This includes providing clinical assessment for patients being placed through the Center for Patient Flow Management. This position is part-time, 24 hours per week, on evening shift (11a-11p or 3p-3a).

• Coordination of incoming patient transfer requests with receiving facility, physicians, and necessary transport resources
• Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes
• Demonstrates clear communication and effective collaboration to optimize patient care and clinical operations
• Displays leadership that influences optimal clinical and operational outcomes
• Delivering exceptional customer service while facilitating communication across multiple disciplines
• Possess and obtain working knowledge of external and inter-facility protocols and standards of practice
• Work with all Center for Patient Flow Management (CPFM) staff in a team orientated, collaborative approach
• Responsible for promoting a Best Place to Work Culture and positive and professional work environment
• Radio communication and coordination with multiple agencies, including Air Medical programs and UH Critical Care Transport
• Dispatch of UH Critical Care Transport ambulances
• Other duties as assigned

Provides safe efficient age specific nursing care to patients/families using the nursing process in order to assist them to attain, maintain, regain, and sustain optimal health status.
