A Brief Overview
Practices at the level of a competent to proficient practitioner of nursing. Demonstrates competence in caring for patients with complex problems, as well as complying with the administrative policies and clinical protocols as outlined in the policy and procedure manuals. In addition, demonstrates clinical leadership skills through contribution to standards of care and division goals. Manages VAD clinical volume, education, patient, and family communications for the Heart Failure & Transplant patient population.
The VAD Nurse Coordinator works collaboratively with the VAD team consisting of and not limited to, Cardiac Surgeons, Cardiologists, Advance Practice Providers, Quality and Safety team and Social Workers to improve patient outcomes by strengthening continuity of care throughout the Cardiac Center. It will include participating in clinical programs and research activities with maintenance of databases, policies, and procedures. Responsible for coordinating on-going care of VAD support of follow up of Heart Failure patients including education of both patients/families as well as the clinical team.

What you will do
  • Practice: Follows Nursing Process to deliver safe care: assessment, planning, implementation, and evaluation
  • Anticipates and synthesizes complex assessment data to best determine and influences total plan of care, pertinent data to be shared with care team, and tasks to be delegated.
  • Collaborates with family, team members and other caregivers to coordinate all facets of patient care, planning with the multidisciplinary team.
  • VAD education for (but not limited to) patients, families, schools, teachers, school nurses, and other medical providers.
  • Collaborate with other health care team members, referring physician and family in planning and coordinating follow-up prior to discharge.
  • Provides nursing support as needed for outpatient clinic visits, inpatient Heart Failure and Transplant services.
  • Daily rounds on pre- and post- operative VAD patients on the CICU (Cardiac Intensive Care Unit) and the CCU (Cardiac Care Unit), wound management.
  • Coordination of VAD-related studies such as ramp studies, exercise studies, and other imaging studies
  • Taking periodic VAD call (carrying family "VAD phone")
  • Management of device stock, in collaboration with Perfusion team
  • Interface with VAD industry representatives for device updates, introduction of new devices, etc.
  • Attending and presenting at national/international meetings related to VAD
  • Participation in academic efforts (quality improvement, writing abstracts, authoring papers, research)
  • Coordinate educational activities for the Cardiac Center Nursing team in partnership with the education nurse specialists.
  • Manages prior authorizations for medications with insurance companies, letters of medical necessity, facilitates transport insurance conversations, and drives other communication with payers and families as needed
  • Routes prescription refills to provider, triages phone calls, communicates with families educating them on the plan of care, and documents all necessary communications

  • Provide an accurate patient status report to members of the team that reflects nursing interventions, patient outcomes and alterations to plan of care.

  • Leads unit improvements and measures outcomes.
  • Assures that staff incorporates professional standards of practice and regulatory requirements into nursing care provided.
  • Research
  • Reads, analyzes, and synthesizes nursing and other scholarly articles and research studies; shares knowledge with unit staff in a formal format.
  • Contacts patients at established intervals, and documents/tracks in database as follow up for clinical, QI (Quality Improvement) and research purposes
  • With assistance applies research findings to clinical practice.
  • Seeks opportunities to participate in research studies or answer a simple research question

  • Identify learning needs of self and others and assist in the development of a learning plan.
  • Applies education and training from departmental in-services and external sources to unit practices to enhance Nursing practice.
  • Assist in developing and utilizing patient education materials relevant to patient population
  • Participates in development/ presentation of staff education programs geared to meet unit and select departmental needs.
  • Demonstrates health care informatics competency to manage patient care, outcome data and current and emerging health care technology.
  • Resource Management
  • Participates in promoting quality, efficient, and cost-effective care.
  • Coordinates resources, providers, and clinical data for weekly clinical conferences, ensuring accuracy, completion of information and organizes flow to achieve efficiency with clinical review meetings
  • Manages own time to accomplish direct/indirect care in a cost-efficient manner.
Education Qualifications
  • Technical Diploma Nursing Required
  • Bachelor's Degree Nursing Preferred
Experience Qualifications
  • At least three (3) years pediatric experience Required
  • At least one (1) year caring for VAD patients Required
  • Active membership in a professional nursing organization required.

  • Ability to perform a literature search on the internet and ability to find professional journal and/or research-based articles
  • Ability to evaluate internet sites for reliability

  • Navigation of electronic health information records and ability to review specific patient data in a clinical information system
  • Understanding of policies and procedures around timeliness of care, use of a clinical information system and support of safe nursing practice

Licenses and Certifications
  • Registered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required or
  • Nursing Temporary Practice Permit (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required and
  • Basic Life Support (BLS) - issued through on organization that requires a hands-on instructor lead psychomotor skills verification (ex. American Heart Association or Red Cross). - upon hire - Required
