Position Summary

The Clinic-Based Pharmacist for Cystic Fibrosis will provide direct patient care in both adult and pediatric CF clinics atMaineHealth. This role is designed to support high-quality, collaborative care through medication management, patient education, and interdisciplinary coordination. The pharmacist will work closely with providers, nurses,nutrition, respiratory therapy, physical therapy, social work, and other team membersto optimize therapy and improve outcomes for patients withcystic fibrosis.They will spend 2-3 days per week in clinic providing direct patient care with the other days spent managing clinical tasks in the pharmacy call-center.This position is supported by "The Green Pod" comprised of the transplant pharmacists and a technician who are trained to cross-cover both cystic fibrosis and transplant patient needs.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: The Clinical Pharmacist role reviews medication orders for safe and effective medication use; collaborates with physicians and other health care personnel to achieve optimal patient outcomes; and formulates recommendations based on current literature and evidence-based practice.
  2. License/Certifications: Current applicable state(s) license with Board of Pharmacy required.
  3. Experience: Previous pharmacist experience preferred.

Key Responsibilities

  • Deliver comprehensive medication therapy management for CF patients in both adult and pediatric clinics.
  • Participate in multi disciplinarycareand collaborate with providers to adjust therapy based on clinical guidelines and patient-specific factors.
  • Educate patients and caregivers on medication use, adherence strategies, and potential side effects.
  • Document clinical interventions and patient encounters using Compass Rose or similar platforms.
  • Support onboarding and training of new pharmacistsas well as pharmacy students and residents.
  • Assist Green Pod members withcoverage needs in the areas of cystic fibrosis and transplant.
  • Contribute to quality improvement initiatives and data collection to support departmental goals.
  • Engage in professional development activities and committee work as appropriate.

Preferred Skills/Qualifications

  • Completion of PGY2 residency program in ambulatory care or pediatrics.
  • Familiarity with CF treatment guidelines and specialty medications.
  • Experience with Epic, Compass Rose, or similar documentation systems.
  • Interest in teaching, mentoring, and contributing to professional development initiatives.
  • Interest in developing and maintaining a collaborative practice agreement with each clinic.
