Make it Happen at Hopkins LOCATION: Onsite @ Johns Hopkins Hospital; Baltimore, MD STATUS: PRN, Casual/On Call (as needed by department) HOURS: M - F, 8:30am - 5:00pm JOB SUMMARY: The Home Care Coordinator Assistant supports the home care coordination team by helping arrange services and equipment needed for patients transitioning from hospital care to home care. In this role, you will help connect patients with home health services, medical equipment, and community resources. You will also assist with administrative tasks that ensure referrals, documentation, and coordination activities run smoothly. It is a beautiful thing when a career and a passion come together. Salary Range: Minimum 16.34/hour - Maximum 26.94/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
