The Intake and Eligibility Specialist will perform initial intake and eligibility assessment of clients seeking services through the Division of Child & Family Services. Assist clients in completion of subsidy application process and verification of documentation.

Duties and Responsibilities



  • Coordinate with Program to ensure program capacity.
  • Communicate with clients via telephone, internet and in person and advise of program requirements.
  • Enter client information into appropriate databases. Ensure all data is entered accurately in all required databases, including Kinderwait, Child Plus,and eCCIMS.
  • Ensure all required documents are included in the child's file.
  • Process all status changes for enrolled children.
  • Complete reassessments as required by the funding source.
  • Develop and maintain relationships with key stakeholders.
  • Assist with the administration and record-keeping of the program.
  • Provide Intake Manager with client tracking numbers as requested.


Successful Candidate will Possess:



  • Proficient computer skills required in basic computer software and data collection systems.
  • Demonstrated ability to prioritize work and problem solve.
  • Excellent customer service, organizational, interpersonal, oral and written communication skills.
  • Ability to maintain accurate records and maintain confidentiality of information.
  • Knowledge and experience working with low income, culturally diverse, and at-risk families and individuals.
  • Bilingual skills (English, Spanish, Khmer) desired.
  • Evidence of recent Mantoux, physical exam, and MMR.
  • CORI/DCF/Fingerprinting.
  • 1-2 years' experience providing intake and referral services, community outreach or other related social service functions.


Education:

HS or equivalent required
