Responsibilities: Join Presbyterian Healthcare Services as an Outpatient Registered Nurse (RN) at our Family Practice Clinic in Socorro. We are seeking a dedicated professional to provide high-quality nursing services for high-acuity patients in a supportive ambulatory care setting. AtSocorro General Hospital, you will impact the local community while enjoying a weekday schedule and a collaborative environment.
- Responsible for walk in and telephone triage.
- Coordinates patients care through collaboration with the patient/family and healthcare team.
- Provides patient care by assessing, planning, implementing, evaluating and documenting individual patient needs, actions taken and patient responses.
- Responsible for case management based on PCP panels, in collaboration with network providers to include nurse visits, telephone management, etc.
- Delegates or assigns aspects of patient care to healthcare team members commensurate with their validated competency to perform the task.
- Documentation within scope of practice.
- Performs other functions as required.
Qualifications:
- State of New Mexico or Compact State Nursing License Required
- Registered Nurse (RN)
- BLS certification required at time of hire
- Minimum ADN preparation with expectation of BSN completion within 5 years of hire
