GENERAL SUMMARY: Under minimal supervision, is responsible for case load/referral management and the overall activities of a specific Henry Ford at Home team. Utilizes the knowledge and application of therapy process and principles to coordinate the provision of high quality, cost effective care. EDUCATION & EXPERIENCE: Requires a minimum of a Bachelor's degree in Physical or Occupational Therapy from an accredited school. Requires current licensure in the State of Michigan as a Physical or Occupational Therapist. Requires a minimum of one (1) year of progressively responsible home health care therapy experience. Progressive experience in program development and coordination preferred. Additional Information
