Cleveland Medical Center (CMC) in Cleveland, Ohio is seeking an experienced advanced practice provider to initiate and lead a geriatric-pre-operative clinic for high-risk older adults who will be undergoing elective surgery at CMC. This clinic is part of the Center's application and participation in the geriatric surgery verification program. Brief Overview: The American College of Surgeons' (ACS) geriatric surgery verification (GSV) program is a quality improvement initiative aimed at enhancing surgical care for older adults. It focuses on improving patient outcomes by addressing age-related factors that can increase surgical risks and complications and is now aligning with the domains of the Center for Medicare and Medicaid's Age Friendly Hospital Measure. UH CMC is currently initiating the application process for the GSV program. One of the requirements for the program is completing geriatric screening preoperatively for adults 65 years of age and older. At the time of referral for surgery at UH CMC, all adults 65 years of age and older will be screened for specific risk factors, including age above 80, cognitive impairment, impaired mobility, and malnutrition. Those who screen positively will be referred to the geriatrics pre-operative assessment clinic, in addition to the usual preoperative assessment through the Center for Pre-operative Medicine. The goal of this clinic will be to address patients' increased risk for complications through deprescribing high-risk medications, addressing goals of care and health care preferences, referring as needed to physical therapy, speech therapy, nutritional therapy, and geriatric medicine, and providing specific recommendations to the surgical teams regarding ways to reduce risk of complications for each patient while hospitalized. The Advanced Practice Provider for this position is responsible for providing quality care for patients and increasing patient access and satisfaction.
