Rutgers, The State University of New Jersey, is seeking a Clinical Care Coordinator in the Department of Pediatrics within the New Jersey Medical School (
NJMS).
The Clinical Care Coordinator (
CCC) supports the Division of Adolescent & Young Adult Medicine (
DAYAM) by delivering sexual and reproductive health services to adolescents, young adults, and their families in both clinical and community settings. As part of a multidisciplinary team, the
CCC is responsible for comprehensive patient care, including assessment, care planning, treatment implementation, evaluation, health education, and coordination of services. The role emphasizes family planning, Pre-exposure Prophylaxis (PrEP) support, Sexually transmitted infections
STI/
HIV prevention and treatment, and advocacy to ensure equitable access to care. The
CCC practices under the clinical guidance of physicians and advanced practice nurses and provides clinical expertise and patient care according to Rutgers protocols and policies and the professional standards of the American Nurses Association. Complies with all regulatory requirements, including
OSHA, Clinical Laboratory Improvement Amendments (
CLIA), Title X,
CDC,
DEA, and the New Jersey Department of Health. Performs administrative and centralized clinic support while promoting a positive work environment.
Among the key duties of this position are the following:
- Works in the DAYAM clinic under the guidance of physicians and APNs and performs needed clinical tasks, including taking histories, distribution/administration of medication, coordination of referrals, pre-authorizations, and other services as needed in collaboration with the healthcare team.
- Facilitates patient flow in the clinic in conjunction with the APN. Takes responsibility for ensuring prompt clinic start to ensure smooth patient flow. Performs triage assessment at the start of each ambulatory care encounter, to include a comprehensive sexual history, mental health, nutritional, substance abuse, and oral health screenings.
- Provides intervention accordingly under the direction of providers /APN, including technical procedures such as phlebotomy, intramuscular, and subcutaneous injections.
- Explains procedures and treatments to the patient to ensure comprehension and obtain consent.
- Provides assistance to providers on intrauterine device (IUD) and contraceptive implant (Nexplanon) insertion and removal, sterile technique, and Long-Acting Reversible Contraception (LARC) visit following protocol required.

Qualifications
| Minimum Education and Experience |
- Bachelor's Degree in Nursing (BSN) or a related health care field.
- Two (2) years of clinical nursing experience required.

| Certifications/Licenses |
- Must have an active licensure as a Registered Nurse in the State of New Jersey.
- Official Family Planning Health Worker training and certification will be provided.
BLS/CPR certification required; maintains current licensure.

| Preferred Qualifications |
- Experience in well woman health and Sexually transmitted infections (STI)/ HIV care preferred.
- Comfort discussing client's sexual needs, family planning concepts, family participation, and interpersonal relationships is highly preferred; experience working with adolescents & young adults and families infected and affected by HIV/AIDS preferred.
- Community health nursing or nursing care of vulnerable families preferred.
- Family Planning Health Worker Certification and Physical assessment skills are preferred.
- Physical assessment skills are essential.
- Bilingual skills (English/Spanish) preferred.
- Proficiency in Epic preferred.
- Excellent research skills preferred.
