The Department of Psychiatry and Behavioral Sciences' Substance Abuse Treatment Programs (SATPs) build upon a harm-reduction framework to integrate addiction medications such as methadone and buprenorphine (Suboxone) with patient-centered, state-of-the-art, supported care.
The Dietitian will provide in-dept nutritional assessment and follow up for ambulatory SATP patients. Develop care plan for nutritional recommendations, which may include instructions on appropriate diet therapies to patients, including considerations for co-morbidities such as diabetes, obesity, etc. Actively participate as core team member in interdisciplinary coordinated patient care program and on committees for the development and revision of nutrition related policies and procedures. Act as a source of nutrition care expertise to MMC staff and associates. This is a part-time role.

Essential Functions:
* Assess patients by evaluating nutritional status through review of the medical record, patient interview including diet history and input from health care team, determining level of care and implementing nutritional care plan
* Complete comprehensive assessment of patients at high nutritional risk including formulation of care plan, goals and recommendations through continuum of care
* Considers all factors related to nutritional status including risk factors from SDOH screen
* Determine appropriateness of current diet prescription and communicates recommendations for change when indicated
* Monitor and evaluates efficacy of care plan in terms of patient compliance, tolerance, laboratory data, and physical and psychological
* Evaluates patient/caregiver need for nutrition counseling. Provides counseling where necessary using method/materials to maximize comprehension and achieve desired outcomes including conformance with diet therapy and enhanced quality of life
* Participate in interdisciplinary conferences and departmental meetings to enhance patient care and provide nutrition expertise
* Act as a resource/source of nutrition expertise to other health care professionals including physicians, and nurses by providing in-services, lectures and instruction as needed/requested
* Maintains current knowledge base through appropriate continuing education to maintain credentialing, current and professional knowledge and expertise
* Educate, instruct and precept dietetic interns in keeping with departmental standards and the role of a major teaching institution
* Actively participates as core team member in all aspects of coordinated care program
* Participate in performance improvement initiatives and other projects in order to provide ongoing improvement in productivity, quality and continuity of patient care
* Actively participates, practices and promotes Montefiore excellence

Qualifications:
* Bachelor's Degree in in Nutrition, completion of qualifying experience and Registered Dietitian (RD) required. Master's degree required if RD obtained after Jan, 2025
* Maintenance of continuing education requirements mandated by the Academy of Nutrition and Dietetics required
* NYS CDN certification within 6 months of hire
* In-depth knowledge of nutritional science and ability to apply it in clinical settings required
* Ability to perform mathematical computations based on clinical evaluation needs required
* Bilingual (Spanish/English) is preferred

The hours and locations of this position are 8:00 am - 3:30 pm @ 3550 Jerome Avenue (1day) & 9:30 am - 5:30 pm @ 2058 Jerome Avenue (2days)
