Position Summary

Serves as a liaison between the patient/family and healthcare team for assessment, coordination of diagnostic work-ups, treatments and services, referrals for outside/contracted services, follow-up of study results, and documentation of the progress of their condition across the continuum of care. This position will focus on the coordination of a patient's care for the patient population of pediatric diabetes, in collaboration with the entire healthcare team, over a prolonged period of time with transition into an outpatient setting. This position will also function as a member of the multidisciplinary CORE team in the outpatient clinic setting to provide/ensure nursing assessments, contribute to the plan of care, coordinate team conferences and provide patient/family education.

Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications.
