Salary: $66,300 - $70,000/yr

Position Summary: The Family Team Conference (FTC) Facilitator will report to the Program Director and work closely with all program staff to schedule and facilitate Family Team Conferences; coordinate meetings with providers to review risks and protective factors as well as service goals and next steps. FTCs will take place initially and throughout the life of the case. The FTC Facilitator will also gather reports on conferencing statistics for the purposes of Quality Assurance (QA) compliance.

Essential Duties:



  • Understand and promote the organization's Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
  • Facilitate Family Team Conferences (FTCs).
  • Conduct pre and post FTC briefing of case with case planner and Supervisor.
  • Identify, evaluate and address all safety and risk concerns during conferences.
  • Oversee the scheduling and coordinating of all conferences, including initial, planning and termination conferences.
  • Collaborate with ACS and other service providers.
  • Prepare statistical reports with regards FTC standards and policies.
  • Ensure adherence with agency, ACS and OCFS policies and procedures.
  • Participate in Children's Aid and ACS meetings, trainings, and planning sessions.
  • Liaison with the CQI department and designated leadership to ensure the program meets all conferencing mandates.
  • Track, record and coordinate all conferences. Document FTC outcomes in Connections and PROMIS within 48 hours.
  • Participate in and conduct ongoing trainings for staff on FTCs.
  • Provide shared coverage for Child Safety Conferences and Family Team Meetings with ACS.
  • Ability to work flexible hours on weekdays and availability on Saturdays, as needed.
  • Perform other duties as assigned.



Minimum Qualifications:



  • Master's degree in social work, mental health, or another related field required.



Key Competencies:



  • Fluent Bilingual English/Spanish preferred.
  • Trained in FTC preferred.
  • Excellent oral and written communication skills (English and Spanish).
  • Thorough working knowledge of ACS Preventive Services systems.
  • Thorough working knowledge of ACS intimate partner violence protocols for FTC.
  • Excellent organizational and management skills.
  • Excellent interpersonal and group work skills.
  • Ability to work under pressure.
  • Ability to connect and have empathy for parents and teens.
  • Experience with families in crisis and at risk of child placement due to neglect/abuse.
  • Knowledge of ACS Connections and PROMIS systems.
