Responsibilities
- Serves as a link between health and social care services and the community to improve access to services and improve the quality of care delivery.
- Develops an understanding of the patient and caregivers' illness understanding and health literacy.
- Follows up on goals of care discussions and ensures all care plans consider the importance of culture and literacy.
- Monitors patient and caregiver needs, including both symptom needs and home needs.
- Works closely with the interdisciplinary team to address changes in patient symptoms and care needs.
- Coordinates patient care, including helping with scheduling appointments and providing reminders and follow-up after appointments/hospitalizations.
- Provides education on the importance of treatment adherence and use of medications.
- Connects patients with community-based resources and assists with screening/enrollment procedures for such resources.
- Provides psychosocial support, which may include informal supportive counseling and motivational coaching.
- Promotes personal empowerment and patient/caregiver engagement in health, including navigation of the healthcare system.
- Promotes literacy around health and social care problems and their management.
- Educates patients and caregivers about the course of illness and helps them understand the disease trajectory and expected outcomes.
- Participates in the weekly IDT meeting with the rest of the care team.
- Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
- Performs other related duties as assigned.
Qualifications
Required:
- High school diploma, GED, or equivalent.
- Two (2+) years of experience working with patients in a home care environment.
- Experience and comfort working in home settings with patients and families, including those in difficult living conditions/environments.
- Experience working in interdisciplinary teams.
Preferred:
- Certified home health care worker or home health aide (depending on local laws and regulations).
- Health coaching certification.
- Palliative care/hospice experience.
- Fluent in Spanish.
