Catholic Charities' Hoarding Intervention and Treatment Program is the only program of its kind in the nation. Offering a comprehensive, holistic approach is to help individuals who have indicators of hoarding disorder to create safe and stable housing. By working along the side of our clients, we help them address the many barriers that come with the disorder. This innovative program provides long-term support by combining case management, in-home coaching, therapy, and community collaboration to improve clients' lives. We empower individuals to regain control of their living spaces and enhance their quality of life. Explore the success stories of our clients here. What Will I Do as a Hoarding Case Manager? In this role, you will: Conduct client inquiries and in-home assessments to understand clients' physical, social, and financial needs. Collaborate with Home Coaches and Therapists to develop individualized care plans for clients. Work with the client one on one in their home, assisting with the environmental de-cluttering work, organizing belongings, and other support in line with the care plan Assist clients in accessing essential services, such as Medicare, food assistance, family care, and disability programs. Monitor client progress by evaluating treatment outcomes, advocating for additional resources, and providing personal support during crises. Qualifications: Bachelor's degree in Social Work, Psychology, Sociology, or a related field. At least six months experience in a social service or health care setting required; 2+ years preferred Prior experience working with older adults, individuals with disabilities, or people with cognitive, physical, or mental health challenges. Valid driver's license, reliable transportation, and vehicle insurance (liability coverage: $100,000/person and $300,000/accident). Salary Range: $45,000 - $47,000 DOE Why Join Us?
