Description

The RN Excel Program Coordinator will utilize the nursing process to provide optimal quality and continuity of nursing care from the pre-operative setting through the complete episode of care. The Program Coordinator will develop a plan of care specific to each patient's needs based on the pre-surgical clinical assessment. The Program Coordinator will work collaboratively as part of a multi-disciplinary team to provide patient and family education, communication with health care providers, and coordination and facilitation of services within the episode of care.

This is a full-time position working Monday-Friday 8:30am-5:00pm out of our Edina location.

Twin Cities Orthopedics is an operating division of Infinite Health Collaborative (i-Health), a group of local medical practices, owned and led by its physicians, on a mission to give patients more control over their healthcare.

Essential Functions:



  • Responsible for managing an orthopedic population throughout the episode of care as part of the Excel Program.
  • Gather clinical information from the EMR and telephonic health assessments to determine risk stratification.
  • Educate the patient and the family of the process and expectations within the Excel Program.
  • Collaborate with multidisciplinary team members in the acute and outpatient settings to identify patient needs.
  • Follow-up on patients at pre-determined intervals and as needed on a patient-by-patient basis.
  • Understand the Excel Program design, implementation, management, monitoring, and reporting requirements.
  • Liaison for Excel Program staffing schedules.
  • Joint Class Educator
  • Remain flexible and responsive when changes occur in patient activity and workload.
  • Adhere to the scope of practice for the Registered Professional Nurse per state regulatory guidelines.
  • Occasionally participate in community functions relating to TCO outreach events throughout the year.
  • Any and all other duties as assigned


Education and Experience Requirements:



  • Graduate of an NLN accredited school of registered nursing.
  • Current licensure as a RN in the State of MN or WI multi-state required.
  • Two years of post-operative care or critical care nursing experience in the last 3-5 years preferred.
  • Current CPR certification required or completed upon hire.

Working Conditions:



  • Frequentlylift up to 50 pounds independently and occasionally up to 100 pounds with assistance.
  • Requires the ability to stand, walk, bend, and move continuously throughout the shift.
  • Must be able to frequently perform physical activities such as pushing, pulling, and reaching.
  • Visual acuity to read electronic and paper documents.
  • Auditory ability to participate in phone or video calls clearly.
  • Manual dexterity to operate standard office equipment (e.g., computer, phone, printer).
