As a Care Coordinator, you'll be an essential part of our clinic team, serving as one of the first and most important points of contact for our patients. You'll help ensure every patient is prepared for their visit while partnering with nurses, physicians, and other healthcare professionals to create a seamless, positive care experience from start to finish. What You'll Do
Maximize clinic schedules by coordinating same-day and future appointments while maintaining at least a 90% clinic utilization rate through proactive scheduling strategies and patient-focused planning. Partner with clinical staff to adjust clinic operations and accommodate same-day or additional patient appointments as needed. Schedule follow-up visits based on provider recommendations and patient acuity. Coordinate appointments with other specialty clinics and provide patients with appointment details, laboratory orders, and other important information. Schedule appointments for patients referred by outside facilities and organizations while maintaining accurate referral records and communicating referral information with the care team. Verify and update patient contact information to ensure appointment reminders and important communications can be delivered electronically, by phone, or through other communication methods. Track, collect, and maintain referral data. Provide training and technical assistance as assigned. Perform routine chart audits to ensure compliance and report findings to leadership. Respond to medical record requests that support patient care coordination. Answer incoming phone calls professionally, route calls appropriately, take accurate messages, and provide clear, helpful communication to patients and staff. Welcome every patient and visitor with courtesy, compassion, and professionalism while creating a positive customer experience. Deliver age-appropriate service that addresses each patient's physical, emotional, educational, and safety needs. Support quality improvement initiatives, patient safety efforts, and a culture that values diversity and inclusion. Maintain knowledge of applicable regulations, organizational policies, compliance requirements, and the Corporate Compliance Plan while participating in required education and compliance activities. Consistently demonstrate honesty, integrity, professionalism, and compassion while supporting the organization's Mission, Vision, and Values. Model exceptional customer service by:
Greeting guests within five feet. Personally escorting guests when assistance is needed. Taking pride in maintaining a welcoming environment. Asking, "Anything else I can do for you?" Ending every interaction with, "It's been my pleasure."
What You'll Bring (Minimum Qualifications)
High school diploma or equivalent required. At least one year of experience in a clerical or administrative role within a healthcare or customer service environment. Computer keyboarding skills of 30 words per minute or greater and basic proficiency with Microsoft Office programs. Excellent interpersonal, verbal, and written communication skills. Ability to read, write, and speak English. Demonstrated commitment to delivering exceptional customer service.
