- High School or GED required.
- Bi-lingual Spanish/English speaking required
- Previous home health care or home care experience highly desirable
- Strong written and verbal communication skills with quick response time
- Ability to work in a fast paced, multi-task environment with competing priorities
- Ability to multi-task and prioritize during high volume, high demand time
- Possess a responsible, detail-oriented, results-driven work ethic
Responsibilities: As a Patient Care Liaison, you will:
- Ensure timely processing of eligibility authorization requests by entering incoming authorizations into appropriate EMR system within established timelines. Verify accuracy of all data points and follow up to resolve any discrepancies or corrections as needed. Monitor Dashboard activities and ensure timely processing of all referrals.
- Support semi-monthly eligibility audit by reviewing assigned client cases for loss of Medicaid or MCO eligibility for assigned clients. Initiate requests for new authorizations for clients with expiring approvals to ensure continuity of care. Track admissions versus declined cases and analyzing trends.
- Conduct welcome calls with new clients, to confirm accuracy of all information, including demographics, emergency contact(s), special needs, insurance coverage, etc.
- Assign the initial caregiver to each case. Confirm schedule details, service address, and assigned coordinator with the caregiver; confirm schedule, caregiver name and permanent coordinator with the client. Ensure a seamless handoff to the assigned coordinator once the case is booked.
- Enter notes from the Coordinators onto the EMR. Partner with internal teams (coordination, EVV, billing, payor relations, etc.) to proactively prevent service interruptions and billing/payroll issues.
