Job Summary

The Loudoun County Health Department is seeking an engaged and community driven Community Health Worker (CHW) Supervisor to help launch a new Community Health Worker (CHW) program within our Population Health team. This is an exciting opportunity to shape a brand-new initiative from the ground up and to build and mentor a team of future Community Health Workers dedicated to advancing health equity and strengthening community wellbeing across our diverse county.

Grounded in culturally responsive public health practice, the Supervisor provides daily leadership and guidance to support high-quality community outreach, strong documentation, and effective workflows. While some activities may appear limited in scope, the position requires extensive preparation, logistical coordination, material assembly, and responsiveness to changing operational needs behind the scenes. The Division Manager will provide feedback based on implementation experience and operational needs and may direct modifications to processes or workflows to ensure alignment with Population Health program goals and service effectiveness.

The Supervisor also coordinates outreach efforts, strengthens community partnerships, and contributes to initiatives that expand access to services and address social determinants of health. Working closely with internal teams and community organizations, the role helps shape strategies that reflect local needs. This position carries direct supervisory responsibilities, including coaching, performance support, and hiring input, and invites a spirit of curiosity, collaboration, and community-centered innovation.

Hiring salary is commensurate with experience.


Minimum Qualifications

Minimum Qualifications:
Bachelor's degree in Community Health, or a related field; plus four (4) years of related work experience in public health, health promotion, social services, mental health services, or related field, including one (1) year of supervision; or equivalent combination of education and experience.

Preferred Qualifications:

  • Considerable knowledge of community capacity-building, partnership engagement, and program development
  • Strong communication, presentation, and relationship-building skills
  • Experience supervising, coaching, or leading staff engaged in community outreach, public health, or client-facing services.
  • Fully proficient in a second language commonly spoken in the community.
  • Experience with grant-funded programs, reporting requirements, and deliverable tracking.
  • Experience developing or delivering community education or staff training.
  • Demonstrated experience developing, implementing, and evaluating community-based outreach, health promotion, or engagement programs serving diverse populations.
  • Demonstrated ability to establish and sustain effective partnerships with community organizations, local governments, healthcare providers, and diverse communities.
  • Community Health Worker (CHW) certification; ability to obtain certification within twelve (12) months of hire may be required.

Job Contingencies and Special Requirements

Valid driver's license and good driving record (driving records may be reviewed annually for continued qualification). Pass pre-employment background checks to include fingerprinting and DMV checks.
