Canvas Health is a non-profit, Certified Community Behavioral Health Clinic (CCBHC) specializing in serving clients who are on Medicaid. Position Summary The role of the MCHN Case Manager is to work in partnership with the other members of the multi-discipline treatment team to address the physical health, mental health and overall well-being needs of assigned clients. This work shall be done in a person-centered, trauma-informed manner that may equal sharing responsibilities across positions at times when client needs arise. The primary responsibility of the Case Manager will be to ensure coordination of care occurs, referrals are completed, and services are monitored over time and to provide therapeutic skills interventions to adults to assist them in engaging in community based mental health, chemical health and physical health care.
