A Brief Overview

This role is responsible for the coordination of assigned community and hospital resource navigation activities within the scope of the Medical Financial Partnership (MFP) program. CHOP Medical Financial Partnership (MFP) is an anti-poverty medicine initiative which acknowledges poverty as a chief social determinant of health and, as such, provides free, high-quality financial services to patient families to increase financial wellbeing and thereby health. Services include financial counseling, tax preparation, public benefits navigation, and
teen financial education, and are mostly provided within the trusted environment of the primary care clinic.

The role functions collaboratively with social workers, community health workers, case managers, health care providers and community partners in clinical and community settings. This individual will be a crucial part of a team dedicated to addressing the impacts of poverty on the health of children, their families, and their community. The role is an engaging and dynamic face of these initiatives in interactions with patients, families, community partners, and other stakeholders. They will actively advance the accomplishment of the MFP mission through patient and partner engagement.

Preference will be given to candidates who demonstrate a specialized knowledge in federal, state, and local public benefits program eligibility and enrollment criteria and processes.

This role will be based out of two of CHOP's West Philadelphia primary care offices, in a hybrid in-person and virtual capacity. They will be primarily responsible for completing duties relevant to respective primary care offices on assigned days, but have flexibility to reallocate time depending on client volumes, scheduling, and other needs.

Establish contact with patients within the hospital setting through referrals from the CHOP teams to raise awareness about information resources, support programs and services, and assist in meeting other treatment-related needs articulated by patients and families.

Responsible for maintaining an ongoing relationship between CHOP departments and resources. Some travel may be required.

What you will do

• Work with patients in the clinical and community setting.
• Establish contact to increase awareness, information resources, support programs and services, and other related needs
• Attends key internal and external meetings and events. Some travel may be required.
• Collaborates and responsible for maintaining an ongoing relationship between internal & external stakeholders
• Evaluates, develops, markets and implements collaborative initiatives for patient population to promote supportive care programming

Education Qualifications

• Associate's Degree Required
• Bachelor's Degree Social Work Preferred

Experience Qualifications

• At least two (2) years experience in the health services field Required
• At least two (2) years of experience in health services, peer navigator role, or community setting providing similar support services Preferred
• Previous counseling experience Preferred
• Previous experience working with chronically ill patients Preferred
• Previous experience in a pediatrics setting Preferred

Skills and Abilities

• Basic knowledge of HIPAA regulations
• Basic knowledge of medical terminology
• Familiarity with electronic medical records systems (EPIC)
• Basic proficiency with office software (Microsoft Office) including word processing and spreadsheet software (Word, Excel)
• Strong verbal and written communications skills
• Strong customer service skills
• Strong interpersonal skills
• Ability to maintain confidentiality and professionalism
• Ability to work independently with minimal supervision
• Ability to convey complex or technical information in an easy-to-understand manner
• Ability to collaborate with stakeholders at all levels
• Ability to be flexible and adaptable to change
