Job Summary 20 hours per week, day shiftThe Palliative Care Nurse Specialist is responsible for providing direct patient care and clinical management for patients and families. The Palliative Care Nurse works collaboratively with the Nurse Practitioner Director of PASCON to provide comprehensive palliative care for patients and families. The Palliative Care Nurse Specialist collaborates with physicians, Mid-level providers, nurses, social workers, and other appropriate staff within the hospital organization, as well as with outside agency staff to ensure team-based, coordinated, comprehensive care and seamless transitions for patients in any setting (hospital, home, nursing home). Does this position require Patient Care? Yes Essential Functions -Works collaboratively with Palliative Care Department Nurse Practitioner Manager to Organize, develop, plan, implement, and evaluate Palliative & Supportive Care Program goals, objectives, policies, and procedures necessary for a comprehensive program, in accordance with hospital policy and appropriate standards. -Provides high quality direct patient care, including education and counseling, to departmental patients in any setting - home, hospital, office, or nursing home. -Performs and records the patient history and physical exam and discusses findings with the NP Manager and physician as needed. -Follows and manages patients (in collaboration with the NP manager and physician) enhancing continuity of care. -Performs, oversees, and assures completion of specialized procedures mutually agreed upon with collaborating physician.
