Regions Hospital Is hiring a Care Management Assistant

Join our Care Management team and help support safe, timely transitions of care for patients. In this role, you'll partner with RN and Social Work Care Managers to coordinate referrals, post-acute and community resources, insurance authorizations, patient and family communication, documentation, and data collection. You'll collaborate with the interdisciplinary care team to identify and escalate barriers that could delay a patient's transition from the hospital.

Work Schedule: FTE 1.0 (40 hrs. weekly), Day Shift, Monday-Friday, No holidays

Required Qualifications:

  • Associate degree in healthcare, human services, business, or a related field, or an equivalent combination of education and relevant experience
  • Previous experience in healthcare, patient-facing care, care coordination, or administrative support
  • No license or certification required

Preferred Qualifications:

  • Bachelor's degree
  • 2+ years of healthcare experience
  • Experience in hospital care management, care coordination, discharge planning support, health unit coordination, medical assisting, patient care, or a related setting
  • CNA, HUC, or other relevant healthcare training or certification
