Position Summary:
We have an exciting opportunity to join our team as a Community Health Rep.

The Center for Healthful Behavior Change within NYU Langone Healths Institute for Excellence in Health Equity is leading a community-engaged maternal health initiative in partnership with the Family Health Centers at NYU Langone Health and NYC Health + Hospitals.

Bridging Maternal Lifestyle Education, and Counseling with Maternal Community Health Workers is a community-engaged research initiative designed to implement an evidence-based lifestyle intervention in partnership with community health workers, clinical teams, and community organizations to improve maternal health outcomes.

The Candidate will play an important role in supporting the implementation and growth of this initiative across New York City. Responsibilities will include assisting with the development and refinement of protocols, scripts, and training materials; supporting outreach and participant recruitment; and helping establish and maintain strategic community and clinic partnerships. The intervention will also involve direct case management, education, and health coaching delivered by Community Health Workers.

Job Responsibilities:

  • Participating in trainings, including nutrition, stress, goal setting, weight management, and outreach.
  • Assisting with development and translation of tailored and translated flyers and other materials.
  • Liaison between pregnant participants, healthcare providers, and the research team at NYU and update about progress.
  • Conducting group and 1:1 sessions on maternal health, including nutrition and weight gain, with pregnant participants.
  • Maintain a caseload of ~25 pregnant participants; assisting participants in tracking medical appointments, and coordination of care.
  • Keeping organized study logs and data files, and tracking of recruitment and intervention activities
  • Providing individual counseling and support; assisting study participant with goalsetting.
  • Assess participant social needs and provide information about community resources and support services.
  • Develop a written action plan that describes how participant needs were identified and addressed.
  • Document appropriate steps taken in the electronic medical record.
  • Enters client data in a web-based platform regularly.
  • Conducting interviews and surveys with study participants
  • Conducting baseline assessments and follow-up phone calls with study participants.
  • Keeping participants motivated and engaged in the study.
  • Adhere to protocols regarding safety and protection of participant health related information

Additional Position Specific Responsibilities:
Haitian Creole Speaker. Computer literate (Microsoft Office), telephone, fax machine, and photocopy machine skills required.

Minimum Qualifications:
To qualify you must have a High School Diploma and minimum of 1 year related experience in an office setting or the equivalent combination of education and experience is required.
Effective Oral, written communication and interpersonal skills.

Preferred Qualifications:
Bachelors degree, preferably in science, public health, health education or a related field. Prior experience in a health care settings. Experience with research studies involving human subjects. Knowledge of Black, Hispanic or other minority health. Knowledge of nutrition and maternal health. Proficiency in using various Microsoft Office applications such as Word, Excel, Access, PowerPoint and Outlook. Familiar with Internet applications. Effective oral, written, communication, interpersonal skills. Ability to work within a team environment as well as independently. Time management skills and ability to work well under pressure. Exceptional organization skills and attention to detail. Must be able to follow study protocol rigorously.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $27.58 - $27.85 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
