Posting Summary

The Health Care Case Manager functions to support the HIV Ambulatory Care Center in meeting the health care needs of infants, children, adolescents, and their families, as part of a multidisciplinary team of healthcare providers. The Health Care Case Manager provides nursing case management services. Nursing case management services include nursing assessment of the client's health status and care needs, development of a health plan, ongoing monitoring and updating of the health plan, treatment adherence monitoring, coordination of care, advocacy, and health education to assure that each patient receives proper health care per the guidelines.
Some key job responsibilities include:

  1. Perform triage assessment and vital signs at start of ambulatory care visit.
  2. Perform initial nursing assessment of patients including mental health, nutritional, substance abuse, and oral health screenings.
  3. Participate in the development of a Plan of Care, together with the multidisciplinary team, documents and re-assess the plan on an ongoing basis.
  4. Provide and document treatment adherence counseling and follow-up.
  5. Assess and update patient financial/insurance information. Assist with ADDP, NJ FamilyCare, and GetCoveredNJ health insurance marketplace applications as necessary.
  6. Assist with referrals to Ryan White related Core Medical and Support Services with follow-up reminders and follow-up to confirm linkages.
  7. Administer medication and performs diagnostics as ordered by licensed providers.
  8. Document all patient care activities and interactions in electronic medical records, C.H.A.M.P., CAREWare and other databases where necessary.
  9. Register patients for clinic visit using the EPIC/Cadence system. Assist with patient flow and customer service issues as they arise in the clinical area.

Required Knowledge, Skills, and Abilities

  • Current licensure as a Registered Nurse in the State of New Jersey required.
  • Bachelor's Degree in Nursing or a BA / BS degree in a health-related field required; may substitute years of pediatric nursing experience for degree.
  • Minimum of 1-2 years of full-time clinical experience as a pediatric nurse in pediatric environments such as, Pediatric doctor's offices, Pediatric Hospital unit, Pediatric public health nursing, Pediatric Emergency department/Pediatric Critical Care unit, Family Practice offices, school nurse, child welfare nursing, Pediatric/Juvenile Mental/Behavioral health facilities, or Children's hospital.
  • Current certification in American Heart Association Basic Life Support for Healthcare Providers required.
  • Vehicle and valid New Jersey license required for off-site visits.


Preferred Qualifications

  • Two (2) years of experience in nursing case management, community health nursing, or nursing of vulnerable families preferred.
