Job Description
The Physical Therapist assesses plans, organizes, and participates in rehabilitative programs that improve mobility, relieve pain, increase strength, and decrease or prevent deformity of patients suffering from disease or injury. Through the use of physical and functional assessments, level of dysfunction is identified; plan of care is determined and documented. This position implements care plan through the use of evidence based modalities and skilled interventions to increase patients' ability to achieve mutually established functional goals. Responsibilities Completes evaluations of assigned patients in accordance with the established standards of care and practice. Plans and provides treatment to assigned patients as appropriate in accordance with established standards of care and practice. Coordinates patient care with other clinical team members ensuring optimal patient care and communication as noted in the clinical record. Educates the patient/caregivers in treatment and care to engage patient in being an active participant in continuing activities at home. Completes all required paperwork following Physical Therapy department policies and procedures to ensure that documentation reflects care provided and the outcomes achieved as evidenced in the clinical record. Monitors and evaluates outcomes related to the assessment and plan of care. Maintains and enhances personal knowledge as well as promoting professional standards in Physical Therapy. Maintains current certification/license. Monitors the treatment activities of PTA's and aides. After one year of experience, may act as student supervisor.Assist in other duties as assigned to support the operational needs of the department and organization. May be required to remain on campus immediately before, during, and after severe weather and/or disasters. Qualifications Minimum Education
