We are currently accepting applications for Homecare Private Duty LPNs. Both part time and full time schedules available, must commit to at least one 8 hour shift per week. Choose where you want to work! Multiple locations available around Columbus:

Provides direct continuous skilled nursing care or skilled nursing visits for individuals and/or groups of acute, chronically ill and terminal patients as prescribed by NCH and community physicians in compliance with regulations as established by the State Board of Nursing, Medicare/Medicaid guidelines, JC standards, and the policies and procedures of Homecare and NCH to patients in the home setting or other environments as approved. The care provided by the NCH LPN shall reflect the highest standards of nursing practice and shall be provided in a manner which promotes quality, continuity and safe patient care. Responsible for providing care for pediatric patients and selected adult patients. Demonstrates competence defined (*) NCHH LPN responsibilities in providing age-appropriate care and service to patients from the age of birth to 21 years, and selected adults.

Responsibilities

* Follows the patient plan of care, as ordered, authorized, and scheduled, under the direction of the Homecare Patient Care Coordinator including, promoting, and providing patient and caregiver education.
* Correctly calculates, prepares, and administers medications by the five rights of medication administration and communicates nursing actions, patient or family responses, and progress toward goals.
* Implements continuous nursing care or skilled nursing visits for patients, demonstrates competence in care and sound clinical decision-making including providing clear and concise documentation of care provided.

* Reviews utilization of service and patient progress, provides input into the revision of care based upon data collected and patient needs, provides instructions, and explains procedures to patients and families. Continues professional growth, possesses knowledge and ability to provide patient education as appropriate.
* Ensures appropriate patient care protocols and quality controls are in place for the delivery, reporting, follow-up, and evaluation of home nursing services. Develops and revises service agreements as appropriate.
* Assures complete documentation for billing, matches practice with authorized allowable with patient needs, provides initial and ongoing advocacy, customer service, and communication including, maintaining confidentiality of all information.
* Performs care for pediatric patients requiring tracheostomies and ventilators. Following additional education, training and demonstrated competency, selected LPN II's may perform venipunctures for the purpose of drawing blood only.



Qualifications

Education Requirement: Must be a graduate of an approved school of practical or vocational nursing.

Licensure Requirement: Must be licensed as a practical nurse in the state of Ohio.

Certifications: CPR certification required.

Skills:
* Strong working knowledge of the concept of family centered care.
* Must have the ability to function collaboratively within the framework of an Interdisciplinary Team.
* Must demonstrate problem solving, organization, communication skills and ability to work independently and react appropriately to crisis situations.
* Basic computer, multi-tasking, customer service, team building, leadership, and decision-making skills.

Experience:
* Must have experience and knowledge in the care of medically fragile children, specifically G/J tube care and tracheostomy care, and a beginning knowledge of ventilator care and maintenance.
* One year of experience in a pediatric care setting and one year of home health experience, preferred.
* One-year previous pediatric home health experience, preferred.
* Two years' experience in a pediatric acute care setting, preferred.
