WHAT YOU WILL BE DOING (ESSENTIAL DUTIES) ...

The Substance Use Recovery Nurse is responsible for providing comprehensive screenings, assessment, care coordination services, addictions/medication assisted treatment (MAT) education and support to patients with Mental Health and Substance Use Disorders. The Substance Use Recovery Nurse promotes overall health and wellness through education, collaboration and outreach activities.

The Substance Use Recovery Nurse is integrated into primary care and integrated behavioral healthcare teams to promote patient-centered care, social determinants of health, increased access to preventative and primary care services, improved health outcomes, decreased hospital admissions and decreased ED utilization.

Essential Duties:

  • Provide a minimum of 20 hours of direct services per week according to program guidelines.
  • Function as the primary worker for a small caseload of clients needing primary skill nursing treatment.
  • Care coordination and/or linking clients to appropriate services.
  • Attend to medication evaluations and psychiatric evaluations.
  • Maintain the clinical record
  • Provide referrals pertaining to medical disorders.
  • Educate clients and family members about medications side effects, and Mental and Substance Use Disorders.
  • Attend multi-disciplinary meetings (MDTP) as scheduled
  • Provide, coordinate and monitor services for all assigned cases. Includes, but is not limited to: face to face and telephone contacts with the client which are documented in progress notes following established procedures, provision and coordination of services as prescribed in the Recovery plan; documented contact and implementation of assigned activities with other client/client family, NMH staff, consultants and Recovery plan- designated staff from other human service and medical organizations, and monitoring the effectiveness of care provided by the agency and through other individuals and agencies by comparing services provided with service outcome, as specified in the Recovery plan.

WHAT WE EXPECT OF YOU...

  • Current licensure to practice professional nursing in the State of Rhode Island.
  • Three or more years' RN experience within substance abuse preferred, which may include inpatient, outpatient, community support, emergency services.
  • BSN preferred.
  • Eligible to be credentialed as a "Qualified Mental Health Professional" through the Department of Behavioral Healthcare, Developmental Disabilities and Hospitals.
