Come and be part of our Home Care team! This is a skilled Home Health position. The HHA would be required to travel from patient to patient within Continuum's service area to primarily provide assistance with bathing and dressing.
Responsibilities include: Follow the plan of care established by the supervising clinician Bathing and Dressing Light Housekeeping Hair Care Assistance with Ambulation/Transfers Position Changing in Bed Oral Hygiene Changing Bed Linens for Incontinent Patients Shaving Skin Care
MINIMUM REQUIREMENTS Education: High School Graduate or Equivalent. Experience: Previous skilled Home Health experience preferred. Licensure: Certified Nursing Aide certification (CNA) through the Virginia Board of Nursing. American Heart Association (AHA) Health Care Provider BLS certification required. PHYSICAL DEMANDS Job requires frequently bending/stooping and driving (Home Health Aide only). Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs and > 100lbs (Home Health Aide only). May be exposed to chemicals, blood/body fluids and infectious disease.
