Sentara Norfolk General Hospital is hiring a Community Health Worker to join the Cancer Services Team.

Come join our amazing cancer team! Along with the nurse navigators and medical social workers, the Community Health Worker will be responsible for the care coordination of cancer patients receiving care in Hampton Roads.

Responsibilities include:

  • Engaging in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illness.

  • Providing education and behavior modification strategies and provides social support to patients and families.

  • Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management.

The Community Health Worker works in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care.

Minimum Requirements:
  • High School Diploma or Equivalent

  • Valid Driver's VA or NC Driver's License

  • Bilingual in English and Spanish is preferred

  • Experience in the community-based setting, human services, oncology, community resources, and care coordination preferred.

  • Knowledge of medical terminology preferred

  • This position may cover multiple locations and requires good organizational and time management skills.
