The Case Manager is responsible for assessing the needs of UACs in care, develops Individual Service Plans, screens for human trafficking concerns, facilitates the safe and timely release or discharge of children and youth, documents the provision of services in case files.

Goal of our Sheltered Hearts Program

Our goal is to provide safe and timely reunification for a child and their family or suitable sponsor.

In partnership with the Department of Health and Human Services, the Sheltered Hearts Program provides family reunification and short-term shelter for children who have entered the United States without a guardian. Children reside in our family-style cottage located on our 2,000-acre campus while our bilingual staff provide a safe and nurturing environment for children.

Job Responsibilities



  • Maintains positive partnership with referring agency including coordinating required services for youth and families, maintaining timely and effective communication regarding case activities.
  • Assists with providing crisis intervention services for youth experiencing acute distress and dysfunctional behaviors to address safety concerns.
  • Provides supportive case work counseling to youth and families including all assessments, treatment and discharged plans.
  • Coordinates psycho-educational groups as needed.
  • Schedules and ensures youth's home visits are goal oriented and work toward building trust, communication and healthy relationships.
  • Facilitate comprehensive treatment reviews according to agency and state regulations.
  • Completes all state mandated and agency documentation according to policy and regulatory requirements.
  • Complies will all federal, state and local laws and regulations, including those or ORR, ICE and the NYS Office of Children and Family Services
  • Assists with ensuring a safe and therapeutic environment including attending various internal and external meetings and case reviews.
  • Prepares court reports and attends court proceedings as scheduled and required.
  • Adheres to agency policies and procedures; follows and supports agency mission, vision and values including participating in internal surveys and data collection for continuous improvement initiatives.



Job Requirements



  • Bachelor's degree in the behavioral sciences, human services or social services fields.
  • Minimum 2-4 years child welfare and/or case management experience is strongly encouraged.
  • Fluent in English/Spanish with excellent verbal and written communication skills
  • Must have a current and valid driver's license with acceptable driving history to the Agency.
  • Must be able to work a flexible schedule including night and weekend hours as necessary.
  • Must be able to be subject to call
  • Varicella Vaccine required OR titers indicating varicella immunity OR documentation from a healthcare provider verifying previous infection.
  • MMR (Measles Mumps & Rubella vaccine required OR titers indicating immunity to all three diseases.
  • Tdap (Tetanus-diptheria-pertussis) vaccine required.
  • Influenza vaccine required, one dose annually during flu season.
  • Physical (Medical Exam) required.
  • Ability to work with economically and culturally diverse population.

    • $25.95 hourly



    Schedule



    • Sunday - Thursday 9 am to 5 pm
