Responsibilities:

1. Complete initial comprehensive assessment of clients who have identified a need for behavioral health services. Assessment requires clinical interview and contact with collateral providers/natural supports/ family members when possible and appropriate and consent is provided.

2. Complete diagnostic formulation and treatment recommendations.

3. Engage client in safety/crisis planning.

4. Provide crisis intervention and de-escalation when indicated.

5. Assess eligibility for services, medical necessity, and treatment preferences.

6. Complete treatment plans including client's strengths, preferences, and behaviorally based goals and objectives as required by program standards.

7. Complete referrals to indicated services and coordinate care on behalf of client, as required by program standards.

8. Document service delivery activities according to program standards

9. Complete consents, releases of information, and other required documentation in compliance with program standards.

10. Complete additional outcome tools, assessments and measures as required by program.

11. Complete reassessments, treatment plan updates, readminister measures, and other documentation at assigned intervals.

12. Attend and participate in multi-disciplinary team meetings and case discussions.

13. Apply culturally competent practices, including gender specific interventions.

14. Participate in required clinical and administrative supervision.

15. Recognize and seek consultation as needed.

16. Complete continuing education activities as directed by licensing body and program requirements.

17. Participate in Quality Improvement Initiatives.

18. Follow all policies and procedures of the agency.

19. Work assigned hours, which may include weekends and evening hours.

Qualifications:

1. Master's degree in a clinical course of study in behavioral health (social work, psychology, marriage and family therapy, counseling, or related field) that meet the academic requirements for independent licensure in psychology, social work, marriage and family therapy, or mental health counseling in the Commonwealth of Massachusetts.

2. Requires licensure as LMHC, LCSW, LMFT, or is actively working towards licensure in the Commonwealth of Massachusetts.

3. Satisfactory CORI background check.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
