Job Summary

The Palliative Care Social Worker is charged with providing direct clinical care to patients and families served by the general Palliative Care Clinic. The Social Worker's primary responsibilities are advanced psychosocial assessment, education, community linkages development, crisis intervention, and psychosocial support for identified patients, families and caregivers. The Social Worker coordinates resources and services in response to individual patients' psychosocial needs. The Social Worker is an active member of the interdisciplinary team, providing person and family-centered perspective in care planning and coordination. The Social Worker also participates in program development and quality improvement initiatives. The Social Worker demonstrates the ability to work with any adult regardless of race, gender, religious affiliation, cultural beliefs, lifestyle, disease process or treatment plan.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The starting rate for MSW: $56.63 (Hourly Rate)
The starting rate for LCSW: $63.43 (Hourly Rate)

Department Description

The UCSF Outpatient Palliative Care Service serves patients with serious illness. Our team includes a multidisciplinary team that works closely with other medical providers to address the patient's and their family's physical, spiritual, and emotional needs to ease or eliminate symptoms. The social workers act as patient advocates by serving as counselors, helping to find resources, and solving problems. Any concerns related to the psychological, practical, and social well-being of each patient or their loved ones may be discussed openly with our social workers.

Required Qualifications

* Master's degree from an accredited school of social work and knowledge and abilities essential to the successful performance of the duties assigned to the position. This position is open to both MSWs and LCSWs. LCSWs must have licensure in good standing.
* Demonstrates resourcefulness, effective written and oral communication, diplomacy, organizational and analytic skills.
* Self-directed, assertive and creative in problem solving, systems planning and patient care management in a high intensity work environment.
* Basic computer skills.
* Ability to work effectively both independently and collaboratively with interdisciplinary team members.
* Demonstrated ability to work effectively with clients from diverse ethnic, socioeconomic, educational, religious and experiential backgrounds.
* Dependable in attendance and completion of work assignments.
* Ability to be flexible and re-order priorities in a changing work environment.
* Commitment to a team approach and to fulfilling the UCSF Care at Home mission.

Preferred Qualifications

* Demonstrated evidence of clinical experience in palliative care, such as outpatient or inpatient palliative care or hospice.
* Three or more years of experience in inpatient, outpatient, or home health based social work preferred.
* Three or more years of experience in providing counseling, crisis intervention and case management preferred.
* Three or more years of experience in working with seriously ill and/or geriatric populations.
* Ability to work sensitively with diverse cultural and socioeconomic groups.
* Knowledge of the social services resources in San Francisco preferred.
* LCSW preferred.
* Excellent oral and written communication skills.
* Proficiency in Spanish or Cantonese.
* Desire to train and work with a variety of allied health professional trainees.
* Experience in program development.
